Отчленение пальцев кисти и их сегментов у детей является более редким видом травм, чем у взрослых. Имеются значительные различия в механизмах травмы, принципах и подходах к лечению детей с данным видом повреждений по сравнению с взрослыми.</p></sec><sec><title>Клиническое наблюдение</title><p>Клиническое наблюдение. Мальчик 15 лет получил травму топором, в результате чего произошла травматическая ампутация кончиков III, IV пальцев правой кисти. Была проведена реплантация обоих отчлененных сегментов. Выполнены шов артерий и нервов, при этом швов вен не проводилось. Результат - приживление реплантированных сегментов.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Основными трудностями, с которыми сталкивается хирург при выполнении реплантации дистальных частей ногтевых фаланг, являются диаметр сосудов менее 1 мм и борьба с венозной недостаточностью из-за невозможности восстановления вен. В связи с этим возникает сложность в послеоперационном ведении больного.</p></sec><sec><title>Заключение</title><p>Заключение. Реплантация отчлененного фрагмента - золотой стандарт лечения детей с рассматриваемым видом повреждений. Важную роль в успешном приживлении отчлененных сегментов играют: правильная их транспортировка, максимально короткий срок тепловой аноксии, качественно выполненное оперативное вмешательство, послеоперационное ведение ребенка.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. Traumatic complete and partial finger amputations are rare in comparison with adults. There are significant differences between mechanism of trauma, principles of treatment children with this type of injury in comparison with adults.</p></sec><sec><title>Clinical case</title><p>Clinical case. A 15-years old child suffered from injury by axe, which caused the traumatic fingertip amputation of III and IV digits of his right hand. Replantation of both amputated parts was performed. Arteries and nerves were repaired with no veins anastomosed. As a result - survival of replanted parts.</p></sec><sec><title>Discussion</title><p>Discussion. A submillimeter diameter of vessels and potential venous congestion are basic problems that a surgeon deal with when perform the replantation of fingertips. Respectively, a difficulty in post-operative care of patient appears.</p></sec><sec><title>Conclusion</title><p>Conclusion. Replantation of amputated part of finger is a golden standard of treatment of children with a described type of injury.
