С.В. Очаповского» (г. Краснодар). При лечении огнестрельных переломов признанным методом является использование аппаратов наружной чрескостной фиксации, однако в современных условиях возможно применение различных хирургических методик одномоментного и поэтапного лечения. Для профилактики инфекционных осложнений пострадавшим с наиболее тяжелыми переломами помимо лекарственных препаратов рекомендуется проведение VАС-дренирования и впоследствии выполнение отсроченной аутодермопластики.</p><sec><title>Цель исследования</title><p>Цель исследования: подобрать наиболее эффективную тактику лечения пациентов с нестандартными клиническими случаями сквозных огнестрельных ранений конечностей.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. При поступлении пациентам проводили лабораторные исследования: общий анализ крови и мочи, биохимическое исследование крови, иммунологические цитологические исследования, рентгенографию и ультразвуковое исследование поврежденной конечности. В дальнейшем поэтапно выполняли хирургическую обработку раны, ее вакуумную терапию, пластику лоскутами на питающей ножке и свободную кожную аутопластику.</p></sec><sec><title>Результаты</title><p>Результаты. В ходе проведенного лечения было получено закрытие сквозных огнестрельных дефектов. В результате поврежденные конечности пациентов были сохранены, имели хороший эстетический вид, а их функции восстановлены.</p></sec><sec><title>Заключение</title><p>Заключение. Ключевыми факторами успешности результатов операции являются последовательность в лечении и активное наблюдение за раневой поверхностью, направленное на раннее выявление потенциальных осложнений в послеоперационном периоде.</p></sec></abstract><trans-abstract xml:lang="en"><p>The article describes clinical cases of successful treatment of gunshot wounds (shot) in the Trauma Department of the Scientific Research Institute – Regional Clinical Hospital No. 1 named after Professor S.V. Ochapovsky" (Krasnodar, Russia). In the treatment of gunshot fractures, the recognized method is the use of external transosseous fixation devices, but in modern conditions it is possible to use various surgical techniques of one-stage and staged treatment. To prevent infectious complications, in addition to medications, VAC drainage and subsequently delayed autodermoplasty are recommended for victims with the most severe fractures.</p><sec><title>Purpose of a study</title><p>Purpose of a study: to select the most successful effective tactics for the treatment of non-standard clinical cases with ligamentous gunshot wounds of the extremities.</p></sec><sec><title>Material and methods</title><p>Material and methods. Upon admission, all wounded underwent laboratory tests: general blood and urine tests, biochemical blood tests, immunological cytological studies, radiography and ultrasound examination (ultrasound) of the injured limb. Subsequently, surgical treatment of the wound, its vacuum therapy, plastic surgery with pedicle flaps and free skin autoplasty were performed in stages.</p></sec><sec><title>Results</title><p>Results. To replace the wound defect with exposure of deep anatomical structures, plastic surgery with skin-fat flaps was performed with an individual approach to the choice of the type of skin plastic surgery.</p></sec><sec><title>Conclusion</title><p>Conclusion.
