Мальпозиция имплантатов является распространенным осложнением после аугментационной маммопластики, она занимает 2-е место среди причин проведения ревизионной хирургии после капсульной контрактуры. Неправильное расположение имплантатов не только эстетически непривлекательно, но и может негативно сказываться на психологическом состоянии и качестве жизни пациенток.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: проанализировать современные тенденции в профилактике и лечении бокового или медиального смещения имплантатов после аугментационной маммопластики.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В базах данных PubMed и Google Scholar был осуществлен поиск статей на английском языке по теме «Профилактика и лечение горизонтальных мальпозиций имплантатов», опубликованных в течение последних 10 лет. В анализ были включены клинические исследования, описания случаев и серии случаев, посвященных данной проблеме. В исследование не включались краткие сообщения, письма в редакцию, обзоры и мета-анализы, экспериментальные исследования. В качестве ключевых слов для поиска необходимой информации и источников использовалось сочетание терминов: «латеральное смещение» и «синмастия после аугментационной маммопластики.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. С использованием ключевых слов «латеральное смещение» и «синмастия после аугментационной маммопластики», а также соответствующих синонимов и вариативных формулировок, было обнаружено 748 статей. При первичном скрининге из поиска были удалены 72 дубликата. Далее мы выполняли отбор статей по их заголовкам, были применены критерии включения и исключения, и из дальнейшего анализа были изъяты еще 608 публикаций. После изучения содержания оставшихся 68 статей, для окончательного анализа были отобраны всего 6 публикаций.</p><p>Горизонтальное смещение имплантатов является относительно редким и малоизученным осложнением аугментационной маммопластики. В исследованиях, в результате отбора вошедших в окончательный анализ, рассматривались боковое и медиальное смещение имплантатов в свете всех мальпозиций, включая верхнюю и нижнюю.</p></sec><sec><title>Заключение</title><p>Заключение. Основным методом профилактики боковых и медиальных смещений имплантатов является разработка оперативного плана на основании индивидуальных особенностей пациентки. Также играют роль размер имплантатов, сохранение складок и фасциальных структур, соблюдение этапов операции, выбор кармана. В качестве лечения бокового или латерального смещения имплантатов авторы предлагают капсулоррафию и применение биологических и синтетических матриц.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. Implant malposition is a common complication after augmentation mammoplasty and ranks second among the reasons for revision surgery after capsular contracture. Improperly positioned implants are not only aesthetically unattractive but can also negatively affect the psychological state and quality of life of patients.</p></sec><sec><title>Purpose of a study</title><p>Purpose of a study: to analyze current trends in the prevention and management of lateral or medial implant displacement following augmentation mammoplasty.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. A literature search was conducted in the PubMed and Google Scholar databases for English-language articles published over the past 10 years on the topic of “Prevention and management of horizontal implant malposition.” Included in the analysis were clinical studies, case reports, and case series specifically addressing this issue. Brief communications, letters to the editor, reviews, meta-analyses, and experimental studies were excluded. The search strategy utilized the terms “lateral displacement” and “symmastia after augmentation mammoplasty,” as well as their synonyms and related combinations.</p></sec><sec><title>Results and Discussion</title><p>Results and Discussion. Using the keywords “lateral displacement” and “symmastia after augmentation mammoplasty,” along with relevant synonyms and alternative phrases, a total of 748 articles were initially identified. After removing 72 duplicates during the preliminary screening, titles were reviewed and inclusion and exclusion criteria were applied, resulting in the exclusion of 608 additional publications. Following full-text assessment of the remaining 68 articles, only 6 met the criteria for final analysis.</p><p>Horizontal implant malposition is a relatively rare and underexplored complication of breast augmentation surgery. The studies selected for final analysis addressed both lateral and medial implant displacement in the broader context of implant malpositions, including superior and inferior displacement.</p></sec><sec><title>Conclusion</title><p>Conclusion. The primary strategy for preventing lateral and medial implant displacement is careful surgical planning based on each patient’s individual anatomical characteristics. Other important factors include implant size, preservation of natural folds and fascial structures, adherence to proper surgical technique, and appropriate pocket selection.
