Комедоновый невус относят к гамартомам. Этиология данного заболевания до конца не изучена, однако в настоящее время его классифицируют как врожденную патологию развития и функционирования волосяного фолликула. Данная патология может существовать как изолированно, так и быть компонентом синдрома комедонового невуса, характеризующегося наравне с патологией развития придатков кожи наличием у пациента аномалий скелета, зрительного аппарата и центральной нервной системы.</p><p>В статье описан редкий клинический случай КН у девочки-подростка, представлены результаты клинического, инструментального и морфологического исследований, а также хирургического лечения.</p><p>Для обследования пациентки применялись дерматоскопия, гистологическое исследование операционного материала, представленного участками комедонового невуса и рубцово-измененной кожи.</p><p>Заключение. Гигантские врожденные комедоновые невусы являются достаточно редкой патологией. Активное рецидивирующее воспаление значительно осложняет лечение или делает невозможным применение общепринятой тактики удаления таких образований с использованием тканевых экспандеров. Лечение с использованием только системной антибактериальной терапии и местных антисептиков не обеспечивает стойкую ремиссию. Наш опыт свидетельствует о том, что рецидивирующее воспаление комедонового невуса эффективно купируется только в случае устранения причины воспалительного процесса хирургическим путем.</p></abstract><trans-abstract xml:lang="en"><p>Nevus comedonicus (NC) is a rare hereditary disease caused by a somatic mutation in the NEK 9 gene, which controls follicular skin homeostasis. Nevus comedonicus should be associated with hamartomas. The etiology of NC is not fully understood, but this disease is currently classified as a congenital pathology of the development and functioning of the hair follicle. This pathology can exist either in isolation or be a component of nevus comedonicus syndrome, which is characterized, along with the pathology of the development of skin appendages, by the presence of abnormalities in the patient’s skeleton, visual apparatus and central nervous system.</p><p>The purpose was to present a rare clinical case of NC in a teenage-girl with the results of surgical treatment, clinical, instrumental and morphological studies.</p><p>A clinical method, dermatoscopy, and histological examination of the surgical material, represented by areas of NC and scarred skin stained with hematoxylin and eosin were used to examine the patient.</p><p>Conclusion. Giant congenital NC are a fairly rare pathology. Active recurrent inflammation significantly complicates treatment or makes it impossible to use the generally accepted tactics of removing such formations using tissue expanders. Treatment using only systemic antibacterial therapy and local antiseptics does not provide stable remission.
