Дефекты I пальца являются сложной проблемой для реконструктивных хирургов, так как этот палец обеспечивает более 50 % функции кисти. В статье предложены варианты закрытия дефектов разной локализации I пальца кисти. Лоскут первой тыльной метакарпальной артерии (в литературе можно встретить его название «Kite flap») позволяет закрывать дефекты, расположенные между анатомической табакеркой и межфаланговым суставом I пальца. В статье представлено клиническое наблюдение: юноша 16 лет с длительно незаживающей язвой по тылу I пястно-фалангового сустава. Выполнено удаление образования. Для закрытия дефекта был использован лоскут на 1-й тыльной метакарпальной артерии. Получен удовлетворительный результат. Впервые лоскут был описан G. Foucher и J.B. Braun в 1979 г., в дальнейшем были предложены различные модификации данного лоскута. Лоскут обеспечивает закрытие дефекта с минимальным ущербом для донорской зоны. Таким образом, применение лоскута 1-й тыльной метакарпальной артерии является эффективным методом для закрытия дефектов основания I пальца кисти.</p></abstract><trans-abstract xml:lang="en"><p>   Objective. Thumb defects are a difficult problem for reconstructive surgeons, as this finger provides more than 50 % of the hand’s function. Options for closing defects of different localization of the first finger of the hand are proposed. The flap of the first dorsal metacarpal artery (in the literature it is called the “Kite flap”) allows to close the defects located between the anatomical snuffbox and the interphalangeal joint of the first finger. The article presents a clinical case: a 16-year-old boy with a long-term non-healing ulcer in the back of the First metacarpal joint. The formation was removed. A flap on the 1st dorsal metacarpal artery was used to close the defect. A satisfactory result was obtained. The flap was first described by G. Foucher and J.B. Braun in 1979, and later various modifications of this flap were proposed. The flap ensures that the defect is closed with minimal damage to the donor area.
