Статистика исходов поражения молнией свидетельствует о том, что ежегодно на планете погибают от удара молнией от 6 до 24 тыс. человек. Смертность при ударе молнией обычно составляет 20–30%, серьезные осложнения и стойкие последствия наблюдаются у 65% выживших. Поражения кожи имеют место у каждого третьего, перенесшего удар молнии. Клиническое наблюдение. Мужчина 27 лет получил травму в результате попадания молнии в лобную область. Была диагностирована ушиблено-рваная рана мягких тканей. Пациент доставлен в участковую районную больницу для оказания первичной медицинской помощи. Через 20 дней после получения травмы обратился в Саратовский областной клинический центр комбустиологии, где ему была выполнена аутодермопластика дефекта покровов черепа.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Прямой удар молнии, как механизм поражения, имеет место при непосредственном контакте пострадавшего с атмосферным электричеством. На его долю приходится не более 5% всех случаев удара молнией. Исход нередко бывает фатальным, поскольку пострадавший получает максимальный разряд электричества.</p></sec><sec><title>Заключение</title><p>Заключение. Аутодермотрансплантация является рациональным вариантом замещения дефектов покровов различной этиологии, в том числе при поражении атмосферным электричеством. Важную роль в оказании помощи в подобных случаях играют адекватное оказание первой помощи, своевременная транспортировка пострадавшего, этапное оказание специализированной стационарной помощи, качественно выполненное оперативное вмешательство, послеоперационная реабилитация. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. The world statistics on lightning strike outcomes reveals 6000 to 24000 human deaths annually. The lightning strike regular mortality is 20–30% with two thirds of survivors having serious complications and long lasting consequences. Every third person struck by lightning bears skin damage.</p></sec><sec><title>Clinical observation</title><p>Clinical observation. The man, 27 years old, was traumatized after the lightning struck into his forehead area.  A contused-lacerated wound of soft tissues was diagnosed. The patient was taken to the local district hospital for primary care. 20 days after receiving the injury, he turned to the Saratov Regional Clinical Center for Combustio- logy, where he underwent autodermoplasty of the defect in the integument of the skull.</p></sec><sec><title>Discussion</title><p>Discussion. Direct lightning strike is technically a result of the immediate contact of the injured with the atmospheric electricity. It comprises no more than 5% all lightning strike cases. The outcome is often lethal as the electrical discharge received is the highest.</p></sec><sec><title>Conclusion</title><p>Conclusion. Autodermotransplanting is advantageous in the replacement of damaged skin integument, inclu- ding that caused by atmospheric electricity.
