В период с 2023 по 2025 г. в ГБУЗ НСО «Государственная Новосибирская областная клиническая больница» 56 пациентам (25 мужчинам (44,6%) и 31 женщине (55,4%)) с лимфаденопатией средостения выполнено эндоскопическое ультразвуковое исследование с тонкоигольной аспирационной биопсией лимфатических узлов. Возраст пациентов варьировал от 27 до 74 лет, средний возраст составил (46,80 ± 12,96) года. У 23 (41,8%) участников исследования были выявлены изменения легочной ткани.</p></sec><sec><title>Результаты</title><p>Результаты. Минимальная длительность вмешательства составляла 15 мин, максимальная 45 мин, в среднем (25,30 ± 6,37) мин. Осложнений во время оперативных вмешательств нами не отмечено. Во всех случаях было получено достаточное количество пункционного материала, у 52 (92,9%) пациентов материал получен при пункции лимфатических узлов средостения. У большинства пациентов (41 человек, 73,2%) был диагностирован саркоидоз. Дополнительно при патоморфологическом исследовании биоптатов легочной ткани и слизистой оболочки бронхов у 16 (28,6%) участников исследовния был диагностирован саркоидоз, в том числе у 5 (8,9%) больных, у которых при пункции лимфатического узла полученный материал оказался неинформативным или была получена нормальная ткань лимфатического узла.</p></sec><sec><title>Заключение</title><p>Заключение. Пункционная биопсия лимфатических узлов средостения под ультразвуковым контролем является высокоинформативной и относительно безопасной процедурой, которая позволила отказаться от диагностической торакоскопии у 91,9% пациентов. Этот метод можно считать золотым стандартом в диагностике лимфаденопатии средостения. Сочетание пункционной биопсии с биопсией слизистой оболочки бронхов и (или) трансбронхиальной биопсией легочной ткани позволяет повысить диагностическую эффективность эндобронхиальной ультрасонографии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose of a study</title><p>Purpose of a study: to evaluate the effectiveness of endobronchial ultrasonography with fine-needle aspiration biopsy of mediastinal lymph nodes in the diagnosis of lymphadenopathy.</p></sec><sec><title>Material and methods</title><p>Material and methods. In the period of 2023–2025, 56 patients (25 men (44.6%) and 31 women (55.4%)) with mediastinal lymphadenopathy underwent endoscopic ultrasound with fine-needle aspiration biopsy of the lymph nodes at the Novosibirsk Regional Clinical Hospital. Patients' ages ranged from 27 to 74 years (mean age (46.80 ± 12.96) years). Lung tissue changes were detected in 23 (41.8%) participants.</p></sec><sec><title>Results</title><p>Results. The minimum duration of the procedure was 15 minutes, the maximum was 45 minutes (mean (25.30 ± 6.37) minutes). We did not observe any complications during the interventions. A sufficient amount of puncture material was obtained in all cases. Mediastinal lymph node biopsy was performed in 52 patients (92.9%), including the majority of patients (41 people, 73.2%) diagnosed with sarcoidosis. Additionally, pathological examination of lung tissue and bronchial mucosa biopsies confirmed sarcoidosis in 16 patients (28.6%), including five (8.9%) in whom lymph node biopsy yielded non-informative material or normal lymph node tissue.</p></sec><sec><title>Conclusion</title><p>Conclusion. Ultrasound-guided mediastinal lymph node biopsy is a highly informative and relatively safe procedure, eliminating the need for diagnostic thoracoscopy in 91.9% of patients. This method can be considered the gold standard in the diagnosis of mediastinal lymphadenopathy.
